A mod crashed while ticking an entity or block — FATAL
A mod's own tick code threw. This is a bug in that mod, not in your setup -- the entity hit a state its author did not handle (commonly a null passenger, vehicle, target or owner). It will usually recur every time that entity exists in that state.
The crash report's -- Entity being ticked -- section names the entity and gives exact coordinates.
Looks random, because it only fires when that chunk loads. It will recur the moment someone goes back.
The thing is still in the world, so the server will die again on restart — this is the classic restart loop. Remove it at those coordinates, or disable that mob in the mod's config.
